IV Compli­cation

Infilt­ration
Extrav­asation
- the catheter breaks of in the veins or air gets into the vain
- blistering due to incorrect medication given or given given correctly
- nursing action: warm compress to allow for vasoco­nst­riction and not lead to more constr­iction
- nursing actions: cool compress for vasodi­lation
 

Nursing actions

- check IV site every hour
- only set mainte­nance fluid for 2 HOURS ONLY worth of volume to prevent overload
- always check IV site from either top down or down up (follow line either from bag to patient or patient to bag)
 

IV Bolus

- dehydrated pt need resusc­itation
- most common is 20ml/kg and will be repeated until they aren't dehydrated (this is volume only)
- follow up with mainte­nance fluid calcul­ation
~ usual fluids are D5 1/2 NS + 20 KCl
- rate for one hour is calculated by taking volume and multip­lying by 1
~ V = 200ml
- 1 hour = 200ml x 1 | 30 min = 200ml x 2 | 20 min is by 3 | 15 min is 4 | 10 min is by 6
- Hollid­ay-­Segar : IV mainte­nance
~ 100 ml/kg for first 10 kg | 50 ml/kg for the next 20 (11-20 kg) | 20 ml/kg for the rest of the weight
~ ie 25kg = (10kg x 100 ml) + (10kg x 50ml) + (5 kg x 20 ml) = 1600 ml/day (24 hours)
~ for per hour 1600ml/24 = 66.7 ml/hour

Indica­tions

- Mainte­nance fluid
- Bolus: if dehydrated or need to give a dose of medication
- Keep vein open is a rate in which the vein will remain open
