Indica­tions

Prevents formation of clots
Pulmonary embolism & thrombus
Drug of Choice For:
DIC

Contra­ind­ica­tions

Beef/Pork allergies or cultur­al/­rel­igious objections
Thromb­ocy­topenia
Bleeding disorder

Cautions

Uncont­rolled HTN: Heparin makes vessels more friable which could result in a fatal bleed
Recent surgery
GI ulcers
 

Adverse Effects

Heparin induced thromb­ocy­topenia with thrombosis (HITT)
Thromb­ocy­topenia

Drug Intera­ctions

Antico­agu­lants
Penicillin
Antihi­sta­mines
Digoxin
Nitrog­lycerin
Nicotine
All of these drugs will exacerbate the effects of Heparin
 

Nursing Interv­entions

Monitor Labs: CBC & PTT
Assess for therap­eutic drug effect & develo­pment of AE
Antidote: Protamine Sulfate
Administer in the event of Heparin overdose
Admini­str­ation Route:
SubQ or IV
Half-life
90 min.
Injection site may form Hematoma
IV site will ooze and have more evident bruising
Drug of Choice for Bridge to other antico­agu­lants such as Warfarin

Labs

PTT
Therap­eutic range = 45-70 sec.
CBC
Monitor for thromb­ocy­topenia (<150)
